link at left to download a blank class book page. You'll need
the free
Adobe Acrobat Reader software to view and print the downloaded
page. Print out two copies - one for practice, one for your final
copy.
Type or write in black ink only. You can also glue text printed
from your computer onto the form.
Print the name by which you were known at Yale in the box in
the upper left hand corner of your page.
Use your "Practice Page" to be sure everything fits
before you do your final page. Remember, your page will appear
in the book exactly as you sent it to us. One-sided only, please!
Glue or tape a current photo in the large box in the upper right
hand corner. the photo needs to be the same size, or smaller than
the box. Unfortunately, we will not be able to return any photos
submitted.
If you have a photo of yourself from your Yale days, glue or
tape it in the small box at the top of the page. Unfortunately,
we will not be able to return any photos submitted.
